PART TIME- 3 days a week 8 am- 5 pm

SUMMARY
Under the supervision of the Behavioral Health Director, the Registered Dietitian (RDN) will provide support to the integrated clinic by providing nutritional assessments, education and management to patients with, or at risk of chronic disease, complications and unique dietary needs. Together, the RDN and patient will develop and implements an individualized care plan designed to promote the patients understanding and management of their condition, optimize quality of life and improve clinical outcomes. The RDN will coordinate with medical and behavioral health providers as it relates to patient’s nutritional needs. This position involves working with community members to assist with accessibility to nutritious foods in in order for patient’s dietary needs being met.

ESSENTIAL JOB FUNCTIONS including the following. Other duties may be assigned:
Consult with providers regarding patients’ diagnosis, prognosis and progress.
Ensures patients and families receive accurate explanations of nutritional assessments and goals for the needed changes and maintenance of health.
Instructs patients and/or families in the proper and safe usage of supplements, formulas, diets and activity.
Monitors high-risk patient progress.
Develops personalized core meal plans for patients based on diagnosis Social Determinants of Health.
Coordinates patient services and referrals with appropriate community organizations when necessary.
Documents Nutrition Care Plans.
Develops and/or approves nutrition education materials.
Acts as a liaison to public, government agencies, and health care providers regarding nutrition and program service issues
Collaborate with community partners and businesses to increase resources relative to nutrition
Participates in the development of policies and procedures.
Assists in planning and coordinating outreach efforts and/or other clinical programs or projects.
Provides nutrition education to patients individually or in groups.
Development and monitoring of Nutrition Care Plans

QUALIFICATIONS

EDUCATION, CERTIFICATES, LICENSING AND/OR EXPERIENCE
Possess a degree from an accredited dietetics program
Possess a registered dietitian nutritionist (RDN) license or certificate
1 year experience in clinical dietetics preferred
