N.O.A.H.’s Foundation Inc. is hiring a highly skilled Program Manager / Lead Housing Case Manager to provide direct, intensive housing case management for individuals and families experiencing homelessness, particularly those with higher barriers or complex needs. This is not a supervisory or administrative role—this is hands-on, field-based client work

ion • Conduct housing-focused case management services, including connection to income, employment, and benef

its • Coordinate and document all services in HMIS (Care/Housing Modu

le) • Actively participate in the CES (Coordinated Entry System) for client matching and referr

als • Assist clients in identifying housing opportunities and overcoming barriers to acc

ess • Conduct field-based visits and mobile case management across Region
