Job Description
The Referral Coordinator is responsible for coordinating specialty care for patients in need of external consultation as requested by the primary care physician. The Referral Coordinator is also responsible for scheduling STAT and Urgent referrals when possible, tracking, and providing reports. The Referral Coordinator represents the company professionally and positively to enhance and promote the core values and mission of NATIVE HEALTH, always exercising utmost discretion, diplomacy and tact in customer interactions. The Referral Coordinator works under the primary supervision of the Nurse Manager and general supervision of the Practice Manager.
KEY ACCOUNTABILITIES:
- Process patient referrals to specialty providers.
- Track referrals.
- Monitor completion of referral.
- Supports the organization as needed.
KNOWLEDGE, SKILLS, AND ABILITIES:
- Knowledge of medical referral practices.
- Ability to communicate clearly in person and in writing.
- Knowledge of various insurance plans including AHCCCS, Medicare, Sliding Fee Scale and private insurance plans.
- Understands the Native American community and culture and has experience working with people from diverse ethnic groups and various backgrounds.
- Must be dependable and be able to prioritize tasks to complete assignments by due dates.
- Ability to maintain strictest confidentiality of patients’ information; adheres to all HIPAA guidelines/regulations
- Excellent customer service skills, including phone skills.
- Proficient in using Electronic Health Records and Microsoft Office (Outlook, Word, and Excel).
- Must be able to function as a team member.
- Ability to work well under pressure, juggle and prioritize multiple priorities, and adjust work accordingly.
Qualifications
QUALIFICATIONS/EXPERIENCE/REQUIREMENTS:
- High school diploma or equivalent.
- Current certification in basic CPR/First Aid training, must maintain a valid CPR card during employment
- Must have a valid and unrestricted Arizona driver’s license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years and no more than two at-fault accidents in the last three years. Must maintain a valid AZ driver license during employment. Must be insurable by NATIVE HEALTH liability auto policy.
- Must pass a criminal background check with a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment.
- Must pass a substance abuse testing upon employment, and submit to a random testing during the course of employment.
- Must have an updated Immunization (IZ) Record.
- Must have a current Tuberculosis (TB) skin test report upon employment and provide an update annually.
