Position Summary

This position supports patients and families on Navian Hawai’i’s Adult Hospice, Pediatric Hospice (Mana’olana Keiki) and Palliative Care Program. Responsibilities include providing psychosocial support, care coordination, and community resource options to participants and their families. The Social Worker is also responsible for routine follow-up through a non-medical care coordination and risk stratification care pathway for participants through requests of community referrals and coordination of provider visits.

Primary Responsibilities

  • Conduct comprehensive bio-psycho-socio-cultural assessments for patients and families receiving end-of-life care; create and implement therapeutic treatment plan
  • Provides crisis intervention, individual, couple and group therapy to patients and families for a diverse range of issues related to grief/loss, adjustment to illness, relationships/family dynamics, mental health, abuse, symptom/pain management, spirituality and bereavement
  • Facilitates case management with referral to appropriate community resources; promotes patient and family advocacy
  • Provides counseling to patients and families regarding end-of-life care issues including medical decision making, Advanced Health Care Directives, disease process and caregiving; initiates community outreach
  • Participates in the provision of social work services in a transdisciplinary care team setting; contributes/participates regularly in IDG team conferences and collaborates with team members in developing a patient centered plan of care
  • Participates in regular clinical supervision with designated Clinical Social Worker and attends Accountability Training
  • Assists in the training and orientation of new hires
  • Performs other duties as required

Knowledge And Skills Requirements

  • Comprehensive theoretical and practical knowledge of individual, group, and family behavior
  • Comprehensive knowledge of the following:
    • Ethical considerations involved in the practice of licensed social work; familiarity with NASW and NHPCO standards
    • Methods, principles, practices, and techniques of assessment, treatment planning and intervention, case management and group work, client-centered advocacy, community organization, research, psychotherapy, and other clinical skills as they apply to hospice patient and family population
    • Individual and group therapy techniques and theories
    • Social, emotional, and financial impact of various health issues on patients, their support system, and the community
    • Availability of community resources and their organization, purpose, and use
  • Possesses the ability to:
    • Apply a variety of techniques and methods in the areas of assessment, treatment, planning, and group therapy, administration, and community organization
    • Analyze and evaluate the effectiveness of various social work treatment and practices, techniques, and procedures; implement treatment plan based on patient and family needs
    • Establish and maintain effective and professional working relationships with Navian Hawai’i staff, clients, family members, the public, and representatives of other agencies and organizations
    • Maintain clinical records and prepare related reports
    • Communicate effectively and professionally with all Navian Hawai’i staff
Experience And Education Requirements

  • Master’s Degree in Social Work (MWS) from an accredited school
  • Valid Social Worker License in the State of Hawai’i
  • 1 year of social work experience required
    • Experience in Hospice or Palliative Care preferred
Employment Requirements

  • A legally compliant background check
  • Drug screen
  • Valid Driver’s License
  • Reliable transportation with valid registration and insurance
  • TB clearance
  • CPR Certification

Working Conditions

  • Indoors in an air-conditioned office as well as outdoors in varying weather conditions
  • Travel to patient homes in a variety of settings

Work Location

This position is based in the field, requiring travel to patient homes, facilities, and offices throughout Oahu using a personal motor vehicle

Hours

  • Generally, Monday through Friday 8am-4:30pm
    • Position may require weekend, night, or holiday availability
Physical, Mental, and Communication Demands

  • Occasional climbing, kneeling, and reaching
  • Requires long periods of sitting
  • Must be able to lift and move up to 25lbs unassisted.
  • Ability to carry 25lbs and push and pull 50lbs unassisted
  • Requires visual abilities such as close vision, depth perception, and the capacity to adjust focus as needed
  • Must be able to work on multiple tasks simultaneously
  • Requires learning many policies and procedures
  • Must be able to speak, read, and write in English
  • Demonstrates skill in written and oral communication, and team building
  • In addition to being able to fulfill the physical, mental, and communication demands listed above, must be able to follow safety instructions and demonstrate critical thinking

Salary Range

$75,000-$85,000 Annually
