Intake Specialist- Home Health Care

8:30am - 5:00pm, Monday to Friday!

We are currently seeking a detail-oriented and dedicated Full-Time Intake Specialist to assist in the coordination of patient care, under the direction of the Executive Director, while fostering Navigate Home Health Care's Values, Goals, and Philosophies.

Responsibilities:

  • Serve as the initial point of contact for all referral sources, patients, and families seeking home health services.
  • Verify insurance eligibility and benefits for prospective patients and coordinate initial authorization processes when necessary.
  • Accurately enter and maintain patient information in the agency's database, ensuring confidentiality and compliance with HIPAA regulations.
  • Assist in communication with patients and families regarding Home Health-related concerns, including conducting welcome calls and verifying demographics, as well as any special requests like gate codes, presence of animals, power of attorney requirements, etc.
  • Monitor Careport, emails, ECINS, and fax machines diligently for new referrals, instantly notifying account executives and the clinical team to facilitate responsive follow-up.
  • Manage weekly reports: Patient Roster for ending episodes needing discharge, Pending Admission, Non-admitted patients, and Medicare eligibility.
  • Coordinate with account executives to gather all necessary patient documentation prior to admission into services.
  • Track pending face-to-face documentation and collaborate with account executives to obtain it in a timely manner.
  • Work with billing to ensure insurance information is accurately entered into the EMR system.
  • Maintain scheduling tracker, pending patients from Hospitals/Rehab
  • Conduct daily calls to facilities for updated statuses on currently hospitalized patients.
  • Collaborate closely with healthcare providers, patients, and families to ensure a smooth and timely admission process.
  • Participate in ongoing training and development to stay current with industry standards and regulations.
  • Collaborate with the scheduler to track pending authorization requests, ensuring all necessary documentation and approvals are obtained promptly to facilitate uninterrupted patient care.
  • Perform other duties, as directed.

Qualifications:

  • High school diploma or equivalent required; Associate degree in healthcare administration or related field preferred.
  • Minimum of 1 year of experience in an intake coordinator or similar role, preferably within a home health or healthcare setting.
  • Proficiency in electronic medical records (EMR) systems and Microsoft Office Suite.
  • Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, and healthcare providers.
  • Strong organizational and multitasking skills, with the ability to prioritize tasks in a high-volume environment.
  • Knowledge of medical terminology and understanding of home health regulations and insurance processes.
  • Must be compassionate, patient-focused, and committed to providing high-quality care.

Job Conditions:

  • Minimal lifting is involved, primarily concerning office records and printouts. The job does not demand extensive physical exertion but does require basic handling of office materials.
  • Visual and Hearing Requirements: Candidates must have the ability to read type of 12 points or larger. Adequate hearing is necessary for effective telephone communication. The use of an amplifier is permitted to fulfill job functions. Clear verbal and written communication skills in English are essential for successful performance in this role.
  • The position may involve high-stress levels due to strict deadlines that must be met consistently. Candidates should be prepared to handle pressure and manage time efficiently.
  • This role is primarily sedentary, requiring long periods of sitting at a desk. It also involves some standing, stooping, and walking. The nature of the job demands a significant amount of telephone communication, necessitating comfort and efficiency in this mode of interaction.
