The LPN Scheduler / Office Nurse plays a key role in coordinating patient care services by managing clinician schedules, supporting clinical operations, and ensuring timely, compliant delivery of care. This role combines scheduling responsibilities with office-based clinical support, requiring strong organization, communication, and clinical judgment.
Key Responsibilities: Scheduling & Staffing Coordination
- Create and maintain daily and weekly schedules for nursing and therapy staff
- Ensure visit frequency compliance per physician orders and plan of care
- Adjust schedules promptly for call-outs, admissions, discharges, and changes in patient condition
- Coordinate staffing coverage to meet patient needs while maintaining productivity expectations
- Communicate schedule changes clearly with clinicians, leadership, and intake staff
Clinical & Office Support
- Review physician orders and plans of care for scheduling accuracy
- Support DON/ADON with clinical coordination and follow-up as assigned
- Assist with tracking missed visits, refused visits, and rescheduled visits
- Communicate with clinicians regarding documentation timeliness and visit completion
- Participate in clinical huddles and coordination meetings as needed
Communication & Coordination
- Serve as a central point of contact between clinicians, intake, and leadership
- Communicate professionally with patients and caregivers regarding visit times when appropriate
- Escalate scheduling or care concerns to DON/ADON promptly
- Maintain clear documentation of scheduling decisions and changes
Compliance & Documentation Support
- Ensure scheduling aligns with CMS, ACHC, and agency policies
- Assist with tracking productivity, visit utilization, and staffing coverage
- Support survey readiness by maintaining organized scheduling records
- Follow HIPAA and confidentiality standards at all times
Qualifications
Required:
- Current LPN license in the state of employment
- Minimum 1–2 years of home health or hospice experience preferred
- Strong organizational and time-management skills
- Ability to manage multiple priorities in a fast-paced environment
- Proficiency with EMR systems and scheduling software
- Excellent communication and problem-solving skills
Preferred:
- Prior experience in scheduling or office-based nursing role
- Knowledge of Medicare home health or hospice regulations
- Familiarity with productivity standards and visit utilization
Physical & Work Environment
- Office-based position with prolonged sitting and computer use
- Occasional lifting of office materials up to 25 lbs
