Position Summary:
(Overall purpose of this position)
- Provides psychiatric evaluation/diagnosis and ongoing treatment to NFP patients in collaboration with primary care and collaborating psychiatrist.
Essential duties and responsibilities which must be performed in order to carry out the position purpose summarized above:

- Performs thorough psychiatric diagnostic evaluations of NFP patients using the biopsychosocial model, complete with diagnoses, treatment recommendations and pharmacotherapy.
- Orders, interprets, and evaluates diagnostic tests to identify and assess patient’s clinical problems and health care needs.
- Provides ongoing medication management/follow-up care needed after the initial psychiatric evaluation, utilizing pharmacotherapeutic and psychotherapeutic modalities as necessary and within scope of practice.
- Develops a diagnosis by analyzing and synthesizing collected data.
- Consults with other with members of multidisciplinary care team and develops a treatment plan, affect change in systems, and enhance the ability of others to provide healthcare.
- Documents all patient data, including patient’s progress in Electronic Medical Record (EMR).
- Provides thorough medication education to patients/guardian about possible risks, benefits, side effects and alternatives to the recommended medication(s) following their psychiatric evaluation in order for them to make an informed consent to treatment.
- Reviews all non-psychiatric medical orders and prescriptions.
- Obtains and reviews patient’s and/or guardian's signature on the Informed Consent for the Proper Use of Controlled Substances upon initial visit and annually thereafter.
- Participates in annual chart review, biannual prescriptive review, regular peer review and performance improvement activities as scheduled and needed.
- Maintains collaborative relationship with physicians in the practice and renders care according to the guidelines of the standard care arrangement.
- Participates in clinic staff development and in-service education programs for both professional and NFP staff.
- Provides consultation and training to NFP providers related to behavioral health issues.
- Serves on NFP committees as assigned.
- Any other assignments as requested by the VP of Behavioral Health & Care Integration.
Qualifications
Qualifications:

- Masters of Science in Nursing or Doctor of Nursing Practice.
- Current RN and APRN Licensure in State of Ohio.
- Board certification as Nurse Practitioner from ANCC or other certifying board.
- Meets credentialing requirements/qualifications for clinical privileges.
- Demonstrated understanding of the scope of nursing practice and the role of the nurse practitioner.
- Demonstrated clinical competence, including health teaching and counseling.
- Demonstrated commitment to continuing professional development and evidence-based practice through independent reading and study, and participation in continuing education programs.
- Demonstrated understanding of public health policy and practices; familiarity with medical home model of care.
- Demonstrated analytical and problem solving skills.
- Demonstrated written and oral communication skills. Ability to communicate in English, Spanish preferred.
- In-depth emotional intelligence: demonstrated self-awareness, accurate self-assessment, sensitivity and empathy, openness, reliability and consistency. Demonstrated cultural competency and understanding of underserved populations.
- Computer proficiency: application of EMR and related functional software.
- Demonstrated collaboration skills.

Previous experience that is necessary background to qualify for this position:
- Moderate level of Psychiatric Nurse Practitioner experience.
