What are the responsibilities and job description for the PATH Outreach Case Manager position at Neighborhood Service Organization?
The PATH Outreach Case Manager, as part of the Tumaini Center Homeless Recovery Services team and in collaboration with the person served, is responsible for providing assessment, case plan development, housing placement and linkage to SOAR services. This position is a community-based position, which involves intensive outreach services. The PATH Outreach Case Manager provides services to an assigned caseload under the supervision of the PATH Outreach Supervisor. REQUIREMENTS: BA/BS/BSW in human services/social work preferred with 2 years experience working with homeless individuals with mental illness required.
Previous PATH experience desired, although related experience will be considered. Social Work license preferred. Valid Michigan driver’s license and proof of auto insurance
