SCOPE OF RESPONSIBILITY
The Direct Care RN non-union is responsible for overseeing the health and wellbeing of the residents and supervision of personnel in regards to medical issues and medication administration under the oversight of the RN Supervisor / Manager.
WORKING RELATIONSHIP
This position reports directly to the Supervising RN. Develops and maintains relationships with the following :
Executive Director
Associate Executive Director
Office Administration
Program Administration
Network, Inc. Employees
External Agencies
QUALIFICATIONS
Valid Nursing Degree / License
Good interpersonal relationships, verbal communication skills
Good judgment and ability to handle confidential information
Ability to handle stressful and / or crisis situations
Good time-management skills (ability to meet deadlines and prioritize multiple tasks)
Must hold a valid CT driver’s license and be insurable with Network’s insurance carrier
JOB RESPONSIBILITIES
Ensure nursing procedures are in compliance with all agency and DDS policies
Carry out the philosophy, goals, and objectives of Network, Inc., while following all governing regulations as they pertain to group homes and intellectually and physically disabled individuals.
Responsible for all medical care for residents in Network’s group homes
Implement MD Orders and ensure that they are followed. Report errors as needed
Order resident medications per MD orders
Administer medications
Monitor residents’ vitals and weights
Coordination of hospital admission and discharge planning of new and current residents in order to ensure that all medical needs are met
Track medical appointments to ensure all medical needs are being met
Attend resident medical appointments
Collaborate with outside medical professionals
Ensure that special diets / consistencies are being followed and prepared properly.
Feed residents and perform ADL as needed
Perform daily review of individuals’ kardexes and MD orders for compliance
Oversee general health and welfare of day program participants
Provide medical assessments as needed for residents
Complete documentation and follow-up as required
File medical documents and maintain orderly medical records for all individuals
Maintain general cleanliness of home and restocking of supplies
Assist RN supervisor when needed
Maintain good communication between residence, day programs, coworkers and supervisors on medical needs of the residents
Ensure program compliance with medical protocol through timely and accurate reporting of medical data, both internally and externally as needed
Perform other duties as required by the RN Supervisor / Manager
EDUCATION AND EXPERIENCE
Graduate of an accredited Registered Nurse Program and maintains a current Registered Nurse license through the State of CT.
