Overview
Phoenix Medical Psychiatric Hospital is looking for detail oriented, compassionate Social Workers to support our patients at our psychiatric hospital. Responsibilities
- Accountable for overall treatment planning efforts, care conferences with families and providers, family and provider care updates, psychosocial needs and completion.
- Coordinate and facilitate the development of a discharge plan of care for the patient population which includes accountability for the complex discharge process.
- Ensure the experience of care for the patient and family is exceptional.
Qualifications
Education:Masters of Social Work (MSW), Counseling, or Psychology required; Psy. D preferred.Experience: Two-Three years of social work experience in inpatient behavior health setting or acute psychiatric settingLicensure: Current Social Worker license or Counseling license in the State of Arizona is highly preferredCPR and Handle with Care certification obtained during orientation.#INDEEDMED
