Seeking a Hospice RN Case Manager Full Time and PRN in the Clear Lake City, TX 77058 and surrounding area!

Responsibilities
- Routine Hospice Visits
- Occasional admissions
- Professional Growth and Development Opportunities
- On-Call/Flexible Schedules
- Documentation of patient’s condition and pointing out significant changes, to be reported to the physician
- Collaboration with our partners and sister companies.
- Collaboration with our interdisciplinary team such as; Social Worker, Physicians, CNA's and Hospice Chaplain.
Requirements
- Texas or Compact Nursing License
- On call
- Strong organizational and self-management skills
- Valid driver’s license
