What are the responsibilities and job description for the Target Case Manager position at New Day Recovery Center?
This position provides services to recovering substance abuse patients in a treatment setting assisting them with goal setting for life management such as housing, employment, transportation, etc. The case manager will connect the patient with resources vital to assist the patient to obtain a successful life style. Approximately 40 hours per week. Mondays, Tuesdays, Wednesdays, Thursdays.
REQUIRED:
-Bachelor's Degree in a human services field (psychology, sociology, social work, family studies, human services, counseling, nursing or another human service degree).
-Minimum of 1 year of related post graduate experience.
