Job Summary

The CCRT Social Worker is a key member of NCST’s Community Crisis Response Team, an

innovative pilot program in Newark, New Jersey. NCST’s CCRT Pilot Program will build off of

the community-response infrastructure already created in NCST. The non-law enforcement,

interdisciplinary community response model will have a centralized response team that

coordinates and co-responds with existing NCST staff that already conducts outreach and

response. Under the 3-year pilot program, NCST’s CCRT will respond to behavioral health and

violence-related crises in the city of Newark experienced by individuals 18 years old and older.

The CCRT Social Worker will be the CCRT lead community responder in the centralized team

and respond to each incident. Working in close coordination with the NCST Dispatcher and

Crisis and Harm Reduction Senior Director, the CCRT Social Worker will also manage the

CCRT Outreach Worker and determine which NCST staff member will co-respond with them to

the incident, depending on the type of level of crisis.

The CCRT Social Worker will provide assessment, de-escalation, and immediate care

coordination, as needed. The CCRT Social Work will work in a variety of Newark settings,

including but not limited to: street outreach settings, homes, homeless shelters, homes, and

other community settings.

Essential Functions

1. Work in conjunction with the CCRT Dispatcher, Crisis and Harm Reduction Services

Senior Director, and fellow NCST outreach staff to respond to behavioral health and

violence-related crises in Newark.

2. Provide trauma-informed crisis response and intervention in a variety of Newark

settings, including but not limited to: suicide assessment and mini-mental assessments

3. Supervise CCRT Outreach Worker

4. Develop clinical protocols and procedures, in coordination with NCST’s Trauma

Recovery Center and Crisis and Harm Reduction Services Senior Director

5. Develop and maintain relationships with community partners

6. Conduct behavioral health assessments and referrals, including to internal NCST teams

7. Primarily responsible for upkeep of CCRT van

8. In coordination with fellow outreach team members, assist in the following areas:

a. Conduct participant intakes and immediate care coordination

b. Conduct onsite case management services, as needed

c. Provide harm reduction education and services

9. Play a consistent and active role in identifying program inefficiencies and finding

collaborative solutions to problems

10. Perform administrative tasks related to the program; participant scheduling, recording

progress notes in Apricot, etc.

11. Coordinate and participate in interdisciplinary case conferences/team meetings

Non-Essential Functions

1. Attend relevant trainings as necessary to maintain professional certification and/or

knowledge.

2. Adheres to NCST’s guidelines and ensures the appropriate handling of sensitive

information

3. May work non-traditional hours based on operational needs and to meet the needs of

the community

4. Completes special projects specific to the function of the department.

5. Other duties as assigned within the scope of position expectations

Experience, Education, and Licensure

Experience: 2-4 years of work experience providing direct social services with high risk

populations; experience in community/outpatient setting preferred. Supervisory experience

preferred.

Education: Minimum Bachelor of Social Work; Masters of Social Work preferred. Licensure

(LSW/CSW) through the NJ Division of Consumer Affairs required
