Job Description:
We are seeking a compassionate and dedicated Outreach Success Coach to connect with individuals experiencing homelessness and provide comprehensive support to help them transition from crisis to stability. The ideal candidate will meet clients where they are—whether at shelters, on the streets, libraries, or other community locations—and work alongside them to navigate housing resources, healthcare, employment, transportation, and other basic needs.
Key Responsibilities:
- Engage with individuals experiencing homelessness in various community settings
- Conduct assessments to identify client needs and develop personalized support plans
- Assist clients in accessing housing solutions, including shelter, transitional, and permanent housing options
- Support clients in obtaining employment, transportation, healthcare, mental health, and substance use treatment
- Provide ongoing emotional support and encouragement throughout their journey toward stability
- Advocate for clients to connect with community resources and services
- Collaborate with shelters, healthcare providers, social services, and other organizations
- Maintain accurate documentation of client interactions and progress
- Promote client independence and resilience, empowering them to thrive
Qualifications:
- Compassionate, non-judgmental attitude, and excellent communication skills
- Experience working with vulnerable populations and in outreach settings preferred
- Knowledge of local housing resources, healthcare systems, and social services
- Ability to work flexible hours, including evenings and weekends if necessary
- Strong organizational skills and ability to handle multiple clients simultaneously
- Familiarity with crisis intervention and motivational interviewing techniques
