Social Work Resource Liaison - Inpatient Psychiatry - Full Time Days

The Social Work Resource Liaison plays a vital role in supporting high-risk patients within the inpatient psychiatry unit by ensuring safe and effective discharge planning. This position serves as a key connector between the hospital and community-based services, coordinating referrals, securing aftercare appointments, and assisting with applications for financial and social support programs. The liaison also escorts patients to community service interviews and arranges transportation when needed. By maintaining up-to-date knowledge of community resources and eligibility requirements, the liaison ensures that patients receive the support they need for a smooth transition back into the community.

Preferred:

• Bachelor’s Degree

• Prior experience working with psychiatric patient population

Required

• High School Diploma or GED

• Proficiency in Microsoft Office Suite

• Knowledge of basic social work principles and practices

• Strong interpersonal and organizational skills with the ability to prioritize tasks effectively
