What are the responsibilities and job description for the Registered Nurse - Weekend (Full-time) position at NEXT CARE HOSPICE?
We are seeking a dedicated weekend on-call RN to join our multidisciplinary hospice team. This is a traditional Baylor-type schedule, where you will be on-call from Friday at 5pm through Monday at 8am. You will be paid a full time salary with benefits for this time frame. The weekend on-call RN will be responsible for triage of any after hours and weekend phone calls as well as subsequent needed visits. You will communicate with referral sources and determine eligibility and appropriateness for weekend hospice referrals as well as any needed admissions. This is a remote position but you must live within LA or Orange County as on-call visits will span both counties.
- Responsibilities
- Collect and record telephonic patient information accurately, coordinate patient on-call visits and potential referrals and visit them timely
- Verify insurance coverage and benefits, and communicate effectively with doctors, patients, healthcare providers, and insurance companies with new referrals
- Assist in the updating of care plans, medications, DME, and any patient changes in the EMR system
- Communicate with physicians after patient assessments using SBAR format, and follow up as needed with patients and families
- Remote QA of hospice EMR as needed when day-time visits are not needed
- Experience
- Previous experience in hospice or palliative care required
- Strong assessment skills and critical thinking skills
- Ability to communicate effectively within a multidisciplinary care team
- Ability to work independently and make appropriate decisions with physician direction in on-call situations
