Duties and Responsibilities:
- Provide applicable physical therapy care according to the specific needs of the patient.
- Proper evaluation of the patient's condition and develop treatment plans according to their needs with specific time frames and achievable goals.
- Accurate documentation of progress notes, charts, referrals and communicate reports to supervisor and other members of the team.
- Consistent updating of the progress of ongoing and new patients; able to gather and analyze information precisely and skillfully
- Provide parent education and training if needed.
- Assist in the proper discharge of patient once goals are established and patient has acquired sufficient benefit from therapy and has no need for further sessions.
- All other duties assigned.
Qualifications:
- License to practice Physical Therapy by the State of California by date of hire
- At least 1 year experience in physical therapy
- Preferably a master's degree from an accredited Physical Therapy program
- Current CPR certification
- Basic knowledge of Microsoft Office
- Possession of excellent interpersonal, time management, problem solving and self-initiation skills
- Preferably willing to relocate or are already located in Sacramento, CA
- Bilingual (English/Spanish) is highly preferred.
