What are the responsibilities and job description for the DD Waiver Service Coordinator position at Nezzy Care of Las Cruces?
Nezzy care is a provider agency that is contracted through the State of New Mexico department pf Health and Developmental Disabilities Support Division. The agency has been providing services to individuals with intellectual and developmental disabilities on the DD Waiver since 2007.
Nezzy Care works hard to ensure that people with developmental disabilities are treated with equality, respect and dignity. The objective of this agency is to educate, support and develop resources in the community to facilitate and provide quality services to individuals we support and their families/guardians in the community based services.
The services are intended to strengthen the individual served by mainstreaming the integrity and dignity of the individual and or family in the least restrictive environment possible through the provision of direct and supportive community based services through personal choice, self-empowerment and person centered planning or maintain skills, opportunities, promoting independence, inclusion and participation in community life. These services are part of on individualized person centered planning process that is implemented through a comprehensive array of services.
Coordinate, facilitate and monitor the provision of DD Waiver services to individuals with developmental and intellectual disabilities. Provide instruction and support with personal hygiene and home cleanliness. Keep quantitative and qualitative data on assigned individuals and ensure required reports and forms are submitted on schedule. Identify natural supports for individual at community activities/functions, promote and facilitate integration, encourage independence to include work related and social activities. Follow Individual Service Plan (ISP), strategies, behavior, physical therapy, occupational therapy, and speech therapy programs. Able to work independently, project a friendly and helpful attitude, ability to work under stressful situations, ability to assess problems and take action.
Oversite duties and ability to handle a caseload of 20 or more depending on availability of new individuals. Able to be (CIE-Job Development) certified Attending ISP and IDT Meetings and managing staff members making sure they are completing all assigned job tasks.
Must have 2 years DD Waiver experience.
Bilingual preferred but not required.
Starting Salary depends on experience. $18.00 without DD Waiver Experience or $21.00 with DD Waiver Experience.
