Ascension at Home is seeking a Full-Time PM Shift RN in the Nashville area!
This position will specialize in providing acute care to patients in their homes through the extensive use of technology, care coordination and clinical pathways. This position will work closely with a care coordination service company and hospital. Responsibilities
Primary functions are to assess, plan, implement and evaluate comprehensive care provided to selected patient populations with the interdisciplinary team including physician, patient/family, and care coordination service; and administer skilled nursing care for clients in their place of residence and assume the responsibility for the total provision of care.
Qualifications
Education/Licensing/Certification:
- Graduate of an accredited college/university, Associate, Diploma, or Baccalaureate School of nursing
- Current, unencumbered State License as a registered Nurse
- Current Driver’s License
- Current CPR, negative TB screen and Hepatitis consent
- 3-5 years of inpatient experience (preferably in med/surg) as a registered nurse within the last 5 years preferred
