Job Summary
Provides home health aide care to patients in their homes, as directed by Clinical Supervisor and Plan of Care.
Duties and Responsibilities
Patient Care
· Works with patient and family; with maturity, compassion, empathy and confidentiality, at all times.
· Follows all written instructions of the Plan of Care and the Agency's policies for Home Health Aides.
· Uses electronic device to record all pertinent care and observations promptly and legibly in the appropriate section of the patient's record.
· Consults and communicates with other members of the Agency's home care team to provide continuity and coordinated patient care.
· Maintains confidentiality and demonstrates respect for all patient/family rights at all times including his/her safety, privacy needs, and property.
· Attends all assigned Inservice training sessions provided by the Agency which includes at least 12 hours annually.
Agency Procedures
1. Always logs in and out of patient visits and completes patient record.
2. Participates in activities and supports agency policies and procedures including staff in-service programs, competency and training skills.
3. Practices Universal Precautions and follows agency policies and procedures for infection control and disposal of hazardous waste where relevant to the position.
4. Reports incidents, accidents, and emergencies promptly. Prepares documentation of these events when requested by agency.
5. Maintains cooperative relationships with agency employees, supervisors, and administration.
6. Complies with employee health requirements, including COVID vaccination.
7. Conveys a positive image of agency when in contact with patients, families, and community at large.
Qualifications
· State Tested Nurse’s Aide (STNA) Certificate or one (1) year experience within the past two (2) years as a Nurse Aide in a hospital, long term care facility, or a qualified home health agency.
· Possess an empathetic attitude toward the care and treatment of patients.
· Ability to read and write and carry out directions; ability and willingness to use agency EMR software in a paperless environment.
· Must have a current driver's license, auto liability insurance, and availability of personal, dependable transportation to conduct and supervise home visits.
· Acceptable references from recent past employers where duties included "hands-on" patient care services as described below.
