As a QIDP at Nobles Group Homes, you play a vital role in supporting individuals with intellectual and developmental disabilities by coordinating care, developing service plans, conducting assessments, and maintaining accurate documentation. Success in this role means proactively managing your caseload, meeting deadlines, following through on services, and serving as a reliable advocate and resource for the individuals you support. Your work directly contributes to the quality of care, compliance, and overall mission of the organization.
Responsibilities
- Responsible for the overall Active Treatment for the individuals receiving services.
- Collaborates with Support Coordinators, Department of Human Resources, Mobile County Public Schools, DMH/DD Regional Office, and other community agencies to ensure individual’s needs are being met as identified by the Interdisciplinary Treatment Team.
- Responsible for the coordination of the individuals Person Centered Plans (PCP’s) and Behavior Support Plans (BSP).
- Works in a collaborative effort in providing BCBA supporting documentation to assist in the development of The Behavior Support Plans (BSP).
- Serves on committees such as IRC, BPRC, and HRC.
- Trains and supervises Plan implementation of programs and the delivery of services according to the PCP and the requirements of active treatment.
- Monitors the collection of data and evaluates the progress toward established goals, conducting periodic reviews to insure program effectiveness and staff diligence.
- Collaborates with QEC/Human Resource Manager to train the direct care staff to provide them with knowledge and skills necessary to serve Individual’s programmatic needs.
- Advocates for each Individual’s specific needs and personal welfare, and insures follow-up when concerns are identified.
- Coordinates the admission process for new individuals, completes initial assessments and program plans.
- Makes arrangements with clinical consultants and external service providers for ongoing evaluations and supports and assures appropriateness of plans submitted by outside consultants and service providers.
- As necessary, transport individuals to and from appointments, meetings, etc.
- Attend individual psychiatric appointments, meetings at MCPSS, and other community Day Programs, and all other appointments as necessary.
- Assures the provision of a generous variety of activities for the Individuals, consistent with the overall mission of the organization and with the individual needs and preferences of each individual.
- Generate quarterly reports for all individuals.
- In addition to above noted tasks and responsibilities other such related duties may be assigned from time to time.
REQUIREMENTS:
The person in this position fulfills the functions of a Qualified Intellectual Disabilities Professional (QIDP) in training under the supervision of the current QIDP. Training QIDP is required to have a Bachelor’s degree in a human services field (including, but not limited to sociology, special education, rehabilitation counseling, and psychology). Advance degree preferred. Valid drivers’ license is a must.
