Job Overview
The Medical Social Worker plays a crucial role in providing support to patients and their families as they navigate the complexities of healthcare. This position involves assessing patients' needs, developing care plans, and coordinating services to ensure comprehensive support. Duties
- Conduct patient assessments to identify social, emotional, and environmental factors affecting health and well-being.
- Develop and implement individualized care plans in collaboration with multidisciplinary teams.
- Provide clinical counseling and psychotherapy to patients and families dealing with mental health issues, including crisis intervention and motivational interviewing.
- Facilitate individual and group therapy sessions, focusing on behavioral health and addiction counseling.
- Ensure compliance with HIPAA regulations while maintaining accurate medical documentation and records.
- Assist patients with intake processes, including diagnostic evaluations and case management.
- Engage in data collection for clinical research initiatives to improve patient outcomes.
- Collaborate with healthcare professionals to coordinate home and community care services for patients transitioning from inpatient or post-acute care settings.
Qualifications
- Oregon LMSW (Masters-level Social Worker) or CSWA (Clinical Social Work Associate) licensure required.
- Valid state licensure as a Clinical Social Worker (LCSW) or equivalent certification preferred.
- Current BLS certification
- Experience as a social worker in a hospital or medical setting preferred.
- Excellent communication skills with the ability to work effectively within a multidisciplinary team environment.
- Demonstrated ability to provide compassionate patient care while managing complex emotional situations.
