POSITION: Intensive Home and Community Based Specialist
LOCATION: Ottawa / Streator, IL
HOURS: Sunday through Thursday 10 AM – 6:30 PM
Reports to: Clinical Manager
Job Summary:
Responsible for providing services and support to adults and youth in their homes and community with basic
needs, medication managements, daily living skills, problem solving and symptom management. Works on a
team providing intensive supports to clients with mental illness.

Duties/Responsibilities:
1. Participates in staffings, meetings, clinical supervision, and trainings
2. Assures sanitary and safe living environments and teaches individuals personal safety and emergency
preparedness
3. Teaches and monitors client’s individual personal care and daily living activities in their home and community.
4. Teaches problem solving, money management and communication skills, provides community support
individual and group services.
5. Plans leisure/recreational activities and encourages and accompanies clients when necessary.
6. Uses technology for service delivery and have the ability to teach clients how to utilize technology to access
services and care support.
7. Completes symptoms management and self-medication training for individuals.
8. Participates with adult community support workers in the identification of skill area deficits for the treatment
plan
9. Closely monitors for physical and emotional safety of clients; and assists with obtaining needed medical care
and acts as a liaison with healthcare providers.
10. Bills and documents services on the treatment plan in order to achieve productivity.

*Requirements

 *
2-5 years’ experience in a social service or healthcare field

Preferred Bachelors or Associates degrees in human services

Possess valid Illinois driver’s license.

Proof of auto insurance coverage and have access to a car during work hours.
