Key Responsibilities

• Engage actively with individuals and families to establish meaningful relationships.
• Document essential information, including goals, shift notes, health changes, and incidents, while upholding confidentiality and reliability.
• Communicate effectively with management and team members, sharing ideas, concerns, and challenges promptly.
• Contribute positively to team dynamics and morale by participating in meetings and maintaining open communication.
• Adhere to organizational policies and procedures while delivering compassionate care.
• Assist individuals in community activities, driving as necessary, while also providing support within their homes.

Qualifications

• Minimum age of 18 years.
• Valid driver’s license and reliable transportation with a clean driving record.
• Completion of a clear and approved criminal record check, as well as a Child Abuse and Neglect Background check.
• Ability to read and write English proficiently.
• High school diploma or GED preferred.
