Brief Job Description/Summary: This is a team lead position that oversees a team of Direct Support Professionals in the provision of direct support and training services to individuals with intellectual, behavioral, and/or physical disabilities

List of Job Duties:

• Provide support, development, and implementation of person-centered planning
• Instruct, supervise, and lead a team of direct support professionals in the provision of person-centered care. Assist in the development of social, leisure, recreational, communication, safety, and other skills in accordance with the person-centered plan.
• -Assist persons served in the development of social, leisure, recreational, communication, safety and other skills
• Plan, prepare, and lead social and educational activities
• Ensure environments are comfortable, productive, and stimulating.
• Ability and willingness to assist with personal hygiene needs, and assist with feeding
• Plan, prepare, and engage in a variety of community-based activities
• Utilize agency and/or public transportation, as needed

• Provide Medication Management and health-related services.
• Provide thorough written documentation of services provided that includes details of staff interventions, and how the consumer responded as outlined by funding source regulations.
• Develop and maintain positive relationships with families, caregivers, referral sources, funding sources and other providers.
• -Maintain records of persons receiving services according to agency policies and procedures and regulatory requirements.
• Report incidents and other safety issues to the supervisor.

Education Level: High School diploma or equivalent
