Case Manager
Job Description
Reports To: Director of Shelter and Equity Services
Signing Bonus: $1500 bonus following successful completion of 60 day orientation period.
Duties and Responsibilities:
Case Management
· Support clients with daily life issues that are barriers to self-sufficiency (such as mental health issues, parenting issues, financial management, scheduling and job seeking, health and nutrition issues, etc.)
· Maintaining files and participating in file review.
· Data Collection and Reporting
Housing Access and Stability
· Work with families on locating apartments they can afford and support them with the transition to their own home.
· Help identify and break down barriers to housing access.
· Support families in document preparation to secure a rental apartment.
· Apply for Rapid Re-housing or other funding to aid families in their transition.
· Conduct 1-, 3-, 6-, 12- and 18-month follow-up aftercare check-in calls.
Trainings
· Conduct weekly training series with topics such as: Nutrition, parenting, renters/landlord rights and responsibilities, financial management, parenting, interviewing, resume building, stress management, as well as other self-sufficiency skills.
· Perform pre and post outcome measures on training sessions.
· Coordinate and schedule outside programming such as Paws for People, Yoga, Project Apples, etc.
Shelter Oversight
· Conduct monthly health and safety checks.
· Conduct monthly fire drills.
· Monitor shelter culture in the milieu and address issues accordingly.
· Perform random drug screens
· Crisis management as needed.
· Building and monitoring a daily schedule
· Upkeep bulletin board.
Screening and Intake
· Manage screening process for family waiting list and clients to be transitioned into the shelter.
· Conduct family intake
· Prepare and ready program for the new family arrival.
Maintenance/cleanliness
· Oversight for the family chore system, as well as, the overall cleanliness of the shelter.
· Participate in the upkeep of the shelter (purchasing, cleaning, repairing, whatever is needed).
On-Call
· Participate in after-hours communication via text /call with clients as needed and team on call communication.
*Other Duties as assigned.
Qualifications:
Education
a. Associates Degree or equivalent experience preferred in; Human Services or Social Work
Language Requirements
a. Bilingual skills (English/Spanish) preferred.
Skills and Abilities
a. Ability to work in a respectful and client centered manner
b. Able to work independently and as a team player
c. Great attention to detail, excellent organizational skills and ability to multi-task
d. Able to communicate effectively and have excellent written and oral communication skills
e. Good working knowledge of Microsoft Office programs
f. Capacity to supervise students and volunteers
g. Working knowledge of landlord tenant issues as they relate to housing of homeless families
h. Satisfactory Criminal Record
Licenses and Other Requirements
a. Obtain and maintain current First Aid and CPR certification
b. Maintain valid driver’s license, acceptable driving record and vehicle insurance coverage
c. Access to a suitable vehicle during working hours
Schedule:
Monday, Wednesday and Friday: 10am-6pm
Tuesday & Thursday 12-8pm
