What are the responsibilities and job description for the Senior Care Coordinator position at NORTH SUFFOLK COMMUNITY SERVICES INC?

Are you passionate about mental health advocacy? Our BH CP program has openings for Senior Care Coordinators to provide person-centered and recovery oriented care management as well as coordination. The team is fun, flexible, and diverse -- working together toward a common goal and sharing our strengths and perspectives. You'll work with care management by identifying goals and developing care plans. The goal is to support those in need to accessing and utilizing resources to improve and maintain their health and quality of life. The focus of the work is less on direct provision of care or service as it is on comprehensive care coordination. 

What You'll Do:

  • outreach and engagement
  • assist Enrollees to identify goals and to develop an individual care plan
  • complete comprehensive assessments
  • facilitate Care Teams
  • coordinate communication among Enrollees, their providers, involved state agencies and others
  • work alongside the Nurse Care Manager, Team Leaders, and others to ensure all aspects of the BH CP services are approved
  • understand and communicate eligibility and referral requirements
  • complete clinical documentation

What You’ll Need:

  • Education: Bachelors in a behavioral heath or social services field.
  • Skills: Valid driver’s license and access to reliable vehicle required.  Bilingualism preferred.  Please indicate language capability in application.  Additional salary differential for program needed languages!
  • Experience:  2 years of experience with working with people with behavioral health disorders required; experience working with people with substance use disorders preferred.
