ESSENTIAL POSITION RESPONSIBILITIES:
· Conduct intakes, provide assessment to determine resident needs.
· Provide home, school, and community visits to assess need and assist in accessing resources.
· Develop a comprehensive service plan.
· Facilitate routine case reviews as well as interagency meetings and red flag reviews
· Create and monitor safety plans for clients when needed.
· Conduct service complaint investigations and retrospective case reviews as needed
· Service complaints & Grievances
· Investigate service concerns as appointment by Executive Leadership
· EBP/Trauma informed care/Models of care
· Communicate all essential referral information in email to all team members for all new admits.
· Provides supportive data, reports, and strategies to program leadership and staff
· PREA Coordinator
· Create court reports for each program resident.
· Attend court when requested by judge, probation, or program leadership
· Weekly progress note submission.
· Routine incident reporting.
· Entry of all client admission and discharge information and documentation.
· Conduct Casey life skill and all other appropriate assessments as identified by program leadership.
· Meet with each resident weekly to discuss progress and reintegration plans with family and community.
· Ensure all educational and routine medical care is coordinated and records requested.
· Facilitate access to additional resources as needed
· Team collaboration with other components of youth and family services
· Create and maintain client files and complete all required documentation
· Provide written or an oral report to office of probation and family court as needed
· Participate in required training
· Participate in staff and program meetings, and in weekly supervision
· Must demonstrate genuine empathy and concern for individuals as indicated in our corporate vision, mission, and values statements.
· Knowledge of culturally competent values, principles and practices required
· Other duties as assigned.
Qualifications
MINIMUM QUALIFICATIONS:
· A bachelor’s degree in social work, criminal justice, human services, or related field
· Two years case management and/or service coordination experience involving high risk youth.
· Knowledge of child and family service systems
· Valid Driver’s License
· Experience with computers beneficial
· Knowledge of social work values and/or child welfare services.
Salary : $45,000
