SUMMARY:
The Medical Care Coordinator’s (LPN) primary responsibilities include identifying individual client’s current medical needs through the completion of Health Screening, receipt of oral records, and/or receipt of medical records to synchronize individualized physical health plans that include internal and external referral sources.
The Medical Care Coordinator acts as a resource to clients and the integrated care team by offering support & referrals to outside providers for care, providing disease management education & follow up, and identifying barriers causing lapses in medical care while simultaneously encouraging and supporting recovery from addiction and/or management of mental health diagnosis.
The Medical Care Coordinator also acts as a client advocate when accessing and navigating health care systems by serving as a liaison between integrated team members & external referral sources. He/she will advocate for clients by providing education on disease process, management, & treatment options while concurrently connecting to resources to help support physical health & well-being.
Qualifications
MINIMUM QUALIFICATIONS:
- Graduate of a Board-Certified LPN Program.
- Current PA LPN Licensure in good standing.
- At least 1 year LPN experience
- Knowledge of mental health and substance abuse systems and supports
- Ability to work independently
- Strong organizational skills
- Ability to and comfort with working with individuals who have serious mental illness and/or substance abuse
