Behavioral Health Community Health Worker
Location: Northern Express Care, Downtown Newport, VT

Northern Counties Health Care is seeking a compassionate and organized Behavioral Health Community Health Worker to join our dedicated team. In this role, you will work in supporting patients by addressing their behavioral health and social needs. Reporting to the Behavioral Health Program Manager, you will play a vital role in coordinating care, providing patient education, and connecting patients with critical resources.
Responsibilities:
- Collaborate with the healthcare team to assess patients’ social, emotional, and financial needs, ensuring a holistic approach to care.
- Use motivational interviewing to support patients’ readiness for change and to help develop self-management plans.
- Provide patients with educational materials and tools for managing chronic conditions and promoting health maintenance.
- Coordinate referrals to behavioral health, community resources, and other social service agencies as needed.
- Facilitate crisis interventions and provide brief, solution-focused behavioral health interventions.
- Maintain accurate documentation of patient interactions and follow-up care within our EMR system.
Qualifications:
- High school diploma or GED required; a Bachelor’s degree in a human services field is preferred.
- Minimum of three years of relevant experience, ideally in primary care or behavioral health.
- Proficiency in motivational interviewing and a thorough understanding of available community resources.
- Strong communication and organizational skills, with a commitment to confidentiality.
- Ability to work both independently and collaboratively within a team-oriented environment.
