ACT CASE MANAGER SPECIALIST - GRAYLING

The Case Manager/SUD Specialist is an advocate and service broker for the consumer; serves as a resource and role model regarding case management services and will serve as the primary Substance Use Disorders (SUD) therapist. The Case Manager/SUD Specialist is supervised by the Operations Manager as assigned. This position has no direct supervisory responsibilities.

Essential Functions
- Approve and coordinate the consumer’s Person-Centered plan. In doing so, attends to the total spectrum of the individual's needs, including but not necessarily limited to, housing, family relationship, social activities, education, finance, employment, health (including special needs), safety, recreation, mobility, protective services and records.
- Monitor the operation of the services that are provided the individual in accordance with the individual program plan, requesting services when appropriate.
- Provide crisis intervention services when appropriate.
- Functions as a primary SUD therapist for consumers with co-occurring disorders.
- Collaborate with consumers (and staff) in assessing their needs, developing treatment goals, objectives, methodologies, timeliness, and evaluating progress toward goals.
- Provide supportive services to the consumer and his/her family.
- Secure relevant data from the other agencies providing services to keep the individual program plan up-to-date and documents according to standards.
- Intervene when necessary to assure implementation of the plan.
- Facilitate the transfer of the individual to another service or agency when such transfer is appropriate to meet the individual's needs.
- Provide services to more clinically challenging cases on the team.
- Serve as a mentor or role model.
Credentialed by NLCMHA to provide services in this professional discipline.
- Masters degree in social work, counseling or psychology
- Bachelor's degree in social work or a degree in Nursing considered.
- Certified in MCBAP as an alcohol and drug counselor or qualify to be placed on a developmental plan through MCBAP
- Three (3) years of post-degree experience in developmental disabilities or mental illness (preferred)
- Experience as a case manager and working with co-occuring consumers is preferable
- LMSW, LPC or Psychologist (preferred)
- Limited licensure considered
