The Case Manager works in depth with patients to help them move toward wellness. Key to the Case Manager’s success is the ability to quickly build rapport with patients. Case Managers engage in assessments to identify patient need, barriers to care and ability to seek and obtain resources independently. Utilizing what they have learned, Case Managers collaborate with patients to create a care plan that connects patients with services, and to collaboratively select meaningful and feasible resources to improve their health. The Case Manager/patient relationship follows a cycle of trust building, assessment, referral, resourcing, and support in an effort to help patients actively participate in their goals of achieving better health outcomes. The Case Manager may work in integration, in one of the pods, providing “in-the-moment” assessment and referral or under a specific program or grant, providing ongoing support to a case load of patient.
