Job Description:


POSITION SUMMARY:

Under direction of the Community Health and Outreach Director, the Community Health and Outreach Registered Nurse (CH/OR RN) provides support for community health planning activities, agency and community relations, resource development for community health improvement projects; and to do related work as required. This position is responsible for working with the community to support community health planning and/or support activities to improve the health of Native Americans in the NVIH service area.


ESSENTIAL JOB FUNCTIONS: The list that follows is not intended as a comprehensive list, but rather to provide a representative summary of the major duties and responsibilities. Incumbent(s) will be required to perform a variety of duties listed, including but not limited to:

  • Provides care management services to patients with complex health care needs and creates a patient-centered self-management plan that ensures access to medical, dental, & behavioral health services and lifestyle interventions to improve health outcomes.
  • Monitors caseloads of other CH/OR team members to provide guidance and support for complex cases; facilities and supports case conference meetings.
  • Facilitates and participates in program quality improvement projects, as well as Government Performance Results Act (GPRA) activities to improve program workflows and patient health outcomes.
  • Provides effective support to patients and works with NVIH staff, and the community to support all phases of public health planning; assessment, planning, implementation and evaluation; applies interpersonal skills to enhance collaboration of NVIH staff, community members and agencies toward common health outcome improvement goals.
  • Contributes to the agency’s effort in identifying top priority health issues for NVIH patients by providing education on the importance of preventative and wellness/health-promoting lifestyle interventions, such as:
    • immunizations,
    • promotion of health screenings,
    • smoking prevention and cessation,
    • nutritional counseling,
    • obesity reduction and prevention,
    • increasing physical activity,
    • substance use prevention/early intervention and harm reduction and
    • promoting independence and skills development related to disease self-management and self-administration of medications.
  • Plan and participates, in collaboration with NVIH staff, to obtain input for community health and outreach planning activities, such as health fairs, health events, and health educational programs.
  • Assists with preparation of grant applications or funding proposals and to promote the development of resources to support innovative population-based health promotion efforts.
  • Other duties as assigned.

Experience and Skills:


The following knowledge, skills, and abilities are required for an employee to successfully perform their duties.

  • Valid and current California Registered Nursing License
  • BSN or PHN certificate preferred
  • Demonstrates clinic knowledge and skills that will be essential in conducting health assessments and developing a patient-centered health plan in accordance with best practice standards.
  • Demonstrates ability to collaborate effectively with multidisciplinary team members and community health organizations to ensure effective coordination of patient care.
  • Demonstrates understanding of age-related or specialty issues and developmental needs for each population served by adapting style, treatment and plan to each population.
  • Ability to use electronic health records and maintain current, accurate medical records, charts and health plans. Familiarity with Microsoft applications, such as Outlook, Excel, Word, & Teams.
  • Proficient in case management skills to support patients and collaborate with providers on complex patient cases.
  • Demonstrates ability to think creatively and critically to support various initiatives
  • Effectively communicate in a clear, concise manner, including but not limited to public speaking
  • Work independently under limited supervision, prioritizes work assignments, show excellent organizational skills, and works well under pressure of deadlines.
  • Ability to travel and operate GSA vehicle for business purposes.
