Summary: As a member of a collaborative team, the SSYI Transitional Coach provides intensive outreach and intervention services and support aligned with the wraparound philosophy and process to help “proven risk” youth engage in and comply with program participation; helps plan, guide, and implement a program of comprehensive prevention and intervention services, and overall evaluation of program participant’ needs. The SSYI Transitional Coach uses best practices that are youth focused and strength-based; works with youth to create individualized strength-based interventions that include informal, natural supports as well as formal services aimed at achieving positive youth outcomes, safe and healthy behavior, and expand their sense of possibility and opportunity.

- Conduct street outreach as directed to locate and engage SSYI referrals from law enforcement.
- Engage and provide case management support to identified participants utilizing tools and practices provided.
- Complete Intake and Risk Assessments to identify participants’ criminogenic factors.
- Create individualized service plans (ISP’s) and adopt S.M.A.R.T. Goals for each participant.
- Perform diligent data entry into the SSYI client relationship manager (CRM) for each participant assigned and maintain accurate documentation of service objectives and outcomes.
- Be available to SSYI Participants for individual consultation and crisis intervention as needed.
- Monitor participant activity related to achieving their personal and program goals, and provide updated revisions to participant service plans, as needed.
- Ensure that participants are knowledgeable of all program resources and advocacy supports available to them.
- Assist in obtaining necessary resources/ documentation for participants to achieve their goals.
- Participant in weekly case management meetings.
- Participate in weekly supervision with supervisor(s).
- Attend program-related trainings /workshops for professional development.
- Serve as an advocate to develop opportunities for SSYI participants in education, workforce development, on-the-job training, and life skills.
- Exhibit professionalism in all job functions, including confidentiality and, establish and maintain effective relationships with all collaterals, including the Courts, Probation, Parole, and New Bedford SSYI Funded behavioral health and reentry partners to meet participants’ goals and performance targets.
- Adhere to strict boundaries and professional ethics in the care of others
- Conducts crisis intervention as needed.
- Commits to continuous growth and learning through consultation, training, and in-service opportunities.
Desired Knowledge and Qualification:
- Familiarity with the New Bedford community and its surrounding towns/cities.
- Understand and relate well to the target youth population/participants.
- Case Management experience.
- Knowledge of and ability to work collaboratively with local agencies and institutions that serve this population (i.e., social services, DYS, DCF, law enforcement, courts, parole, corrections, and others).
- Possess strong communication and interpersonal skills.
- Able to prioritize tasks, multi-task when necessary, and work independently in the absence of supervision.
- Experience with Microsoft Office.
Requirements:
- Minimum education and experience:
- Minimum High School Graduate or Equivalent, Post HS education preferred
- Three years of counseling, social work, or related experience, one year of which must have included work with youth considered at high risk of school failure.
- Licenses and certifications:
- Current certification in first aid and CPR preferred; certification within six months of employment required.
- Valid driver’s license and access to a registered and insured vehicle with the following minimum amounts of liability coverage: $100,000 injury per person; $300,000 injury per accident; and $5,000 property damage.
- Satisfactory criminal history background checks as determined by the agency.
- Evidence of compliance with agency and state health requirements
KNOWLEDGE, SKILLS, and ABILITIES:Knowledge of:
- Wraparound philosophy and process.
- Basic functioning of school systems.
- Program goals and objectives, procedures, and reporting requirements.
- Best practices in home visiting.
- Principles of positive youth development and best practices in assets-based youth development programming.
- Nature, dynamics, and impact of child abuse and neglect and other trauma issues.
- Adult learning styles.
- Effective parenting techniques.
- Available community resources, including formal services and informal supports.
- Cultural competency.
- Systemic racism and racial equity as it pertains to youth of color, ages 17-24.
- Ability to identify and address the impacts of racism, trauma and individual/community violence.
Skilled in:
- Verbal communication, including the ability to speak before groups and the ability to interpret the program. Fluency in Spanish, Crioulo, and/or Portuguese as well as English a plus.
- Written communication, including the ability to record observations and prepare moderately complex reports. Literacy in Spanish and/or Portuguese a plus.
- Use of personal computers and related software applications.
- Basic knowledge of crisis intervention techniques.
- Interpersonal communication, including the ability to respond warmly and sensitively to youth, families, staff, and other persons of diverse experiential, educational, cultural, and socio-economic backgrounds.
Ability to:
- Work as a member of a team.
- Establish and sustain effective working relationships with staff of a wide variety of agencies and institutions.
- Identify, organize, and appropriately schedule the multiple responsibilities associated with the job.
- Conduct normal duties without close supervision.
- Exercise good judgment and provide appropriate intervention in unexpected or emergency situations. Working conditions and physical effort:
- Ability to work flexible hours, including responding to nighttime and weekend emergencies as well as overnight and out of state field trips.
Physical ability to:
- Drive to and access all locations where families may be living.
- Hear and see at a distance required to identify and address safety issues.
- Move quickly to supervise and assist youth or, on an infrequent basis, to prevent injury or harm to a youth.
- Be absent from work no more than the typical adult to provide continuity with youth and families.
