What are the responsibilities and job description for the Family Assessment and Planning Team (FAPT) Case Manager position at Northwestern Community Services Board?

This position is responsible for managing an assigned caseload of children and adolescents experiencing primarily mental health problems. The FAPT Case Manager caseload is primarily non-Medicaid clients that have been referred by private/state hospitals, local FAPT teams, and other community collaterals, including their guardian. The incumbent is responsible for assessing client needs, developing, implementing, and reviewing service plans, and working with other community resources (FAPT teams, state/private hospitals, and residential facilities) in meeting and achieving client service needs.

QUALIFICATIONS:

  • A combination of mental health work experience or relevant education, and training, which indicates the possession of knowledge, skills, and abilities necessary to perform essential job duties required.
  • Bachelor’s degree earned from an accredited college or university in relevant human services field required.
  • At least one year of experience working with children required.
  • Qualified Mental Health Professional (QMHP) eligibility required.
  • Must possess a valid Driver’s License, good driving record, and the ability to provide your own transportation.

ADDITIONAL REQUIREMENTS:

  • May be exposed to allergenic environments from client’s homes, including dust, smoke, pet contamination, cleaning supplies, etc
  • May be exposed to clients with infectious diseases.
  • Must be able to drive for long periods of time to travel in and out of the catchment area as needed.
  • Must be able to walk distances, climb and descend stairs, maneuver in and out of agency vehicles.
  • Prolonged periods sitting at a desk and working on a computer; and
  • Must be able to lift/push/pull up to 25 pounds at times

SALARY: Starting at $52,788

RETIREMENT: NWCSB participates in Virginia Retirement System.
