Northwestern Counseling & Support Services is seeking a dynamic team member to support adolescents in Franklin and Grand Isle Counties. This position provides clinical behavioral health case management services to encourage positive youth development.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Interviews clients with challenges such as personal and family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments to determine degree and nature of problems.
- Secures information such as medical, psychological, and social factors contributing to youths situation, and evaluates these and their capacities.
- Counsels clients individually, in family, or small groups regarding plans for meeting needs, and aids client to mobilize inner capacities and environmental resources to improve functioning.
- Helps clients to modify attitudes and patterns of behavior by increasing understanding of self, personal challenges and youths contributions to problem solving.
- Refers clients to community resources and other service organizations.
- Compiles records, prepares reports and attends meetings outlined in program requirements.
- Develops individual plan of care, reviews service plan and performs follow-up to determine quantity and quality of service provided to support transition and discharge.
- Accesses and records client and community resource information.
- Secures supplementary information such as employment, medical records, or school reports.
- Delivers (or assists in delivering) Life Sills Instruction to individual/groups.
- Provides outreach to youth and youth focused community partners.
- Participates in outreach activities and all Adolescent Services team meetings, activities and supervision as assigned.
- Complies with all agency policies and paperwork requirements.
EDUCATION and/or EXPERIENCE:
Bachelors Degree; or equivalent combination of related education, training and experience.
Salary : $37,500 - $47,500
