Responsibilities
The Pharmacy Infusion Navigator-RN serves as an expert clinical consultant to hospital staff and a clinical educator and advocate for patients, with the goals of raising awareness about the benefits of home infusion and optimizing the transition of care for patients referred to Norton Home Infusion. As a clinical expert, you will educate patients, physicians, case managers, and all customers regarding disease states, infusion therapy, transitional care management, and other Norton Home Infusion programs and services. You will facilitate timely placement of discharge patients requiring home infusion or enteral services by gathering preadmission data and collaborating with hospital discharge team, including initial patient set-up/training and ongoing patient/caregiver education. A portion of responsibilities will be spent interfacing with healthcare professionals and hospital staff in an effort to strengthen partnerships and grow business through referrals. Candidates are nurses with excellent clinical, interpersonal and communication skills. Must be able to work independently and effectively manage tasks in a fast-paced environment. Must be a team player with a positive attitude who works well in a cross-functional, matrixed environment.
Qualifications
Required:
- Minimum of three (3) years clinical (RN) nursing experience
- Bachelor Degree
- Active Compact/Multistate Registered Nurse (RN)
Desired:
- Minimum three (3) years working in health care or pharmacy organization
