Job Title: Licensed Vocational Nurse (LVN)
Department: Home Health Agency
Reports To: Clinical Supervisor / Director of Nursing
Location: Upland, CA

Position Summary:
The Licensed Vocational Nurse (LVN) provides direct patient care in the home setting, ensuring the delivery of high-quality, compassionate healthcare services to patients under the supervision of a Registered Nurse (RN) or physician. The LVN will be responsible for administering treatments, medications, and various clinical procedures while monitoring and documenting patient progress.

Key Responsibilities:

  • Patient Care:
  • Provide skilled nursing care, including administering medications, wound care, and treatments as prescribed by the physician.
  • Monitor patient vitals and clinical symptoms to assess their health status and report findings to the supervising RN or physician.
  • Assist with patient mobility, hygiene, and daily living activities as needed.
  • Educate patients and families about proper health management, medication adherence, and self-care techniques.
  • Clinical Responsibilities:
  • Assist in creating and implementing care plans under the guidance of the RN.
  • Perform delegated clinical tasks such as catheter care, specimen collection, and respiratory therapy assistance.
  • Administer injections and IV therapies as per protocols and physician orders.
  • Document patient care activities in the electronic medical record (EMR) accurately and in a timely manner.
  • Patient Monitoring and Documentation:
  • Regularly monitor patient progress, reporting any changes in condition or response to treatment to the RN or physician.
  • Maintain accurate and up-to-date patient records, including vital signs, medications, and patient condition.
  • Ensure that all clinical documentation complies with regulatory standards and agency policies.
  • Collaboration and Communication:
  • Collaborate with RNs, physicians, and other healthcare professionals to ensure comprehensive patient care.
  • Communicate effectively with patients and their families, addressing concerns and providing support as needed.
  • Participate in team meetings to discuss patient care plans, concerns, and progress.
  • Compliance and Safety:
  • Ensure patient safety and compliance with all infection control policies and protocols.
  • Maintain knowledge of and adhere to all relevant healthcare regulations and agency policies.
  • Participate in ongoing education and training to ensure knowledge of best practices and compliance with industry standards.

Qualifications:

  • Education:
  • Graduation from an accredited LVN program.
  • Licenses/Certifications:
  • Current, valid LVN license in the state of [State Name].
  • Current BLS (Basic Life Support) certification.
  • IV certification (if required by the state).
  • Experience:
  • Prior experience in home health, hospice, or other healthcare settings is preferred.
  • Experience working with elderly patients or individuals with chronic illnesses is a plus.

  • Strong clinical and critical thinking skills.
  • Ability to work independently and manage time effectively.
  • Excellent communication and interpersonal skills.
  • Knowledge of medical terminology, procedures, and home health care regulations.

Working Conditions:

  • Travel to patients' homes is required; a reliable mode of transportation is necessary.
  • Flexibility to work varied hours, including weekends or evenings, may be required based on patient needs.
  • May be required to lift or assist patients with mobility as part of daily tasks.
