- Assess needs, plan and manage all aspects of our comprehensive activities program, working with department resources, team members, and volunteers
- Get to know each resident individually to better understand needs and varying interests and coordinate activities accordingly
- Completing screens and orientating residents and families to Oak Park Place
- Helping residents and families adjust to the center
- Assisting families and residents with financial questions and referrals

We are currently looking for an Social Worker for our beautiful Nakoma facility. We are looking for a motivated individual to oversee our Social Services Department in a Skilled Nursing and Assisted Living Community
Requirements:
- Associate degree preferred
- Previous experience in a supervisory role
- Previous experience working with seniors is preferred
- Strong organizational and planning skills required
- Ability to work independently is a must
Salary : $51,000 - $64,600
