Job Mission:
To connect newly diagnosed or out of care individuals living with HIV to medical care and support services.
Performance Standards:
- Perform outreach to identify People Living with HIV/AIDS (PLWHA) and provide linkage to care and health education.
- Develop relationships with federally defined points of entry to market services and facilitate referrals.
- Participate in quality improvement initiatives for assigned programs.
- Provides health education and risk reduction counseling, information about available resources and services, and psychosocial support to clients as they access the continuum of care.
- Carry out all other duties as assigned by supervisor.
Qualifications:
- High School Diploma or equivalent required.
- Minimum one (1) year experience working with HIV populations preferred.
- Must obtain Michigan Community Health Worker certification within one year of hire.
- One (1) year of experience working with computer applications especially using Windows environment for word processing, data base, spread sheet applications and client data systems.
- Valid Michigan Driver’s license; active insurance and reliable transportation to be used for work related travel required.
