POSITION OVERVIEW

 

The Assisted Living Shift Coordinator provides leadership, support, and oversight to assisted living Resident Assistant staff.  This position coordinates and provides medical and emergency support for residents of the CBRF in compliance with appropriate codes, with an emphasis on promoting wellness and independence.  Provides support to Assisted Living staff and oversees operational and customer service issues and requests per the Assisted Living Director’s directive.

This position is accountable to the Director of Assisted Living and Memory Care.

ESSENTIAL RESPONSIBILITIES (including but not limited to)

 

  • Responds to symptoms of change in mental or physical conditions of residents, provides services to residents that meet their physical, emotional, intellectual, social and spiritual needs, and reports these changes to the Director of Assisted Living and Memory Care, Nurse, and/or AL Care Team designee.
  • Provides oversight of Resident Assistants promoting teamwork and ensuring adherence to Oakwood Village policies. Routine rounding to visualize and verify.
  • Audits medication carts routinely, processes reorders through pharmacy.
  • Ensures Resident Assistants are passing and recording the administration of scheduled and PRN medications and treatments as required. Reports all medication errors to the Director of Assisted Living, Nurse, and/or designee and fills out appropriate forms.
  • Provides assistance and training as needed to Assisted Living staff in maintaining resident charts and records.
  • Provides assistance to residents with Activity of Daily Living (ADL’s) to promote independence on a daily basis as part of the Oakwood team.
  • Reports significant resident incidents or injuries to physician and responsible person as they occur.  Then sends follow-up reports to the appropriate clinical team members. Provides emergency services to CBRF and RCAC residents, as needed.  Notifies the appropriate people, e.g., on-call nurse, family, AL Care Team. Requests RN assistance as needed.  Fills out incident reports as required.
  • Attends appropriate staff development sessions for development and growth. Attends staff meetings and other meetings as assigned.
  • Participates in discussion and development of resident's care cards to support individual care plan/ISP; assures that Resident Assistants are familiar with and providing services/support as indicated on each resident's care plan/ISP.
  • Works with Health Services Staffing Coordinator to insure proper coverage for their shift. Notifies Director of Assisted Living and Memory Care of any possible Oakwood attendance policy or call-in violation for follow up. 
  • Supports nursing staff in providing for a smooth transition for new admissions for resident and family.
  • Maintains good communication with clinical nursing staff, Director of Assisted Living and families/responsible persons.
  • Facilitates orientation/education/training of new staff members and provides ongoing support as needed.
  •  Motivates and encourages staff to reach their optimal performance level.

     

ESSENTIAL QUALIFICATIONS

  • Possesses a genuine caring and ability to relate well to residents and families.
  • Demonstrates knowledge and skill in current personal care practices, especially relating to a geriatric population.
  • Plans, organizes, and prioritizes their workload to meet deadlines and perform work in a timely manner.
  • Effectively builds and maintains teamwork relationships with other supervisory/leadership staff.
  • Has open, transparent communication and provides work direction and training effectively to the Resident Assistants.
  • Performs job responsibilities in a professional, accurate, and conscientious manner.
  • Maintains and protects confidential information regarding residents and staff members.
  • Effectively exercises independent judgment and makes sound decisions.
  • Demonstrates deep knowledge of DHS 83 and DHS 89 codes.

 

EXPERIENCE, EDUCATION, AND/OR TRAINING

  • N.A Certified or ability to obtain.
  • Minimum of 1 year supervisory/management experience preferred.
  • CPR certified (can certify on the job).
  • Proficient in Microsoft Office 365.

Experience with electronic medical documentation preferred.
