Education, Experience, Minimum Qualifications:
- Graduate of an accredited school of nursing as a Registered Nurse
- Two years' experience as a Registered Nurse
- Healthcare Provider CPR
- Previous clinic experiences preferred
- Process Improvement experience preferred
- Accountable Care Organization experience preferred
- Ability to work independently as well as with a team
- Ability to be flexible and to work well under pressure
- Strong computer skills; PowerPoint, Epic, Excel and other visual presentation programs
- Good Verbal and written communication skills
Job Summary:
This position works closely with the Senior Director of Clinics and the Clinic Supervisor. Must have the ability to work in a self-directed manner, with initiative and objectivity. This position will demonstrate organizational and interpersonal skills while fostering productive, positive relationships with staff members and providers. Guide and assist the clinic nurses and CMA staff in the daily operations of the clinics. In addition to this responsibility; the process for training and assessing the competencies of nurses and CMAs in the clinics, coordinating clinical workflows, and liaison with Epic Informatics as the super user for nurses and CMAs. This person functions as an integral member of the clinic leadership team.
- Triage patient calls and ensure timely follow-up for care needs
- Review hospital discharge and emergency department visits for appropriate follow-up while coordinating inpatient discharge follow-up needs with hospital MSW
- Consult with provider to provide effective and compassionate care
- Provide support to assistants and providers through in-basket management
- Serve as a care coordinator for patients when needed by consulting with providers and scheduling follow-up appointments
- Monitor outcomes for success and effectively solve any obstacles for patients
- Participate in committee meetings for clinics and ACO needs
- Responsible for CMA and nursing clinic competencies
- Responsible for the onboarding of new assistants
- Able to step in as an assistant for a provider if needed
- Provide guidance and oversight to nursing staff, including RNs, LPNs, and MAs
- Assign duties, monitor workflow, and ensure adherence to clinic policies
- Conduct staff training and competency assessments for new hires and annually for nursing staff
- Work closely with providers and administrative staff to enhance workflows and promote high-quality patient care outcomes
- Address patient concerns, complaints, and service improvements in collaboration with supervisor and risk management
- Perform nurse appointments for patient/caregiver education, injections, vitals monitoring, suture removal, wound care, etc.
- Monitor clinic wide health trends and help identify care gaps, implementing outreach to promote improved patient outcomes
- Responsible as the EPIC superuser to keep staff informed of upcoming updates to the EHR system and utilization
- Serve as needed as a care coordinator, communicator, and quality improvement facilitator for the continuous improvement of OBHMC's ACO program.
- Assist in developing workflows that support the goals of the ACO.
- Monitor for successful outcomes and effectively problem solve any obstacles.
- Participate in committee meetings.
- Develop and maintain standard work documents in support of Medicare Annual Wellness Visit workflows.
- Act as the Medicare Annual Wellness Visit "super user" and provide ongoing training and support for staff and providers.
- Responsible for nursing care delivered to assigned patients during a Medicare Annual Wellness Visit.
- Monitor data to ensure trends and plans meet targeted goals.
