Job description

Responsibilities :

• Develop, implement, and assess patient nutrition care plans.
• Conduct age-appropriate evaluations for patients identified as being at nutrition risk, including diabetes management and education according to established guidelines.
• Customize nutrition plans based on patients’ unique economic, social, cultural, and age-specific needs.
• Regularly reassess patients and modify care plans as necessary.
• Provide tailored education to patients and families to support their nutrition care plan.
• Evaluate learning needs and readiness to learn for patients and families.
• Contribute to the creation and review of educational materials and resources to address the needs of the patient population.
