Job Description Scope of Position This specialty practice pharmacist position acts in an interdisciplinary setting to provide comprehensive pharmaceutical care and disease management. The position serves to provide clinical leadership for outpatient pharmacy ambulatory care clinics, clinical support to the patient care team, conducts patient education, facilitates prior authorizations, assists in prescription capture and dispensing, and collaborates closely with The Ohio State University Outpatient Pharmacy while providing efficient and high quality care for our patients. Position Summary This position functions to coordinate all aspects of patients' pharmaceutical care. Candidate must be proactive in fulfilling duties which include but are not limited to: assurance of appropriate dose, route, frequency, and duration of therapy; patient medication profile review for drug-drug interactions, duplication of therapy, drug-allergy interactions and appropriate regimens; coordination of patients' pharmaceutical needs with physicians, nurses, and pharmacy; communication with insurance companies and specialty retail pharmacies regarding prior authorizations and prescription coverage; writing appeal letters if prior authorizations are denied, optimizing clinic operations to streamline both clinic and specialty pharmacy workflows, and developing evidenced-based treatment notes for the patient care team. Must be able to demonstrate pharmaceutical knowledge and skills necessary to provide care to adult and geriatric patients. This position is also responsible for supervising pharmacy residents and students, technicians, undergraduate and graduate students, and providing educational programs for hospital staff (physicians, nurses and pharmacy staff). Duties and Responsibilities Provides direct patient care in assigned ambulatory clinics, in compliance all Ohio State Board of Pharmacy and The OSUWMC policies, regulations, and laws. Maintains patient medical profiles, completes and maintains clinic specific competencies, effectively navigates the electronic medical record and other technology to perform job. Dispenses medication pursuant to a physician's order according to the laws and regulations as defined by the Ohio State Board of Pharmacy and The OSUWMC drug distribution policies. Enters or reviews entry of medication orders into the pharmacy computer system. Duties include but are not limited to: medication order approval and distribution; assurance of appropriate dose, route, frequency, and duration of therapy; patient medication profile review for drug-drug interactions, duplication of therapy, drug-allergy interactions and appropriate regimens, patient counseling, disease state management, and coordination of patients' pharmaceutical needs with physicians and nurses. Monitors the accuracy of drug dispensing. Fulfills all applicable MUE requirements. Complies with OBRA 90 requirements and accreditation standards applicable to pharmacy practice and employment. Percentages may vary during first years of employment. Minimum Education Required Bachelor's Level Degree Required Qualifications Registered pharmacist in the State of Ohio; advanced degree (Pharm.D./MS), residency training, Board Certification and/or equivalent experience preferred.
