Minimum Qualifications:
Level I:
- Possession of a valid permanent Oklahoma license as a registered nurse as approved by the Oklahoma Board of Nursing or Registered Nurse Licensure Compact (eNLC)
- Two years of professional nursing experience
- Possession of a valid permanent Oklahoma license as a registered nurse as approved by the Oklahoma Board of Nursing or Registered Nurse Licensure Compact (eNLC)
- Three years of professional nursing experience
- Identifies assigned service recipients health concerns and consults with the case manager to assist in facilitating identification of health risks with recommendations prior to the annual Individual Planning (IP) meeting on the following task below, 90% of the time
- Completes an Health Review and update Medical Event, Diagnosis, Allergy, Medication and Provider section in CCM
- Analyzes data from documentation such as home records, CCM, DDS-5, DDS-6, Incident Reports, IP, agency and electronic communications to assist case managers in identifying health risks/concerns of service recipients
- Consults and submits written Health Review to the case manager with recommendations to develop the Health Section of the IP
- Develops a Nursing Service Support Plan as indicated
- Emergency PASRR assessments are submitted to state office within 5 calendar days
- Routine and Reconsideration PASRR assessments are submitted to state office within 7 working days
- Initial 24 Hour Mortality Report completed within 24 hours of notification of service recipient’s death and submitted to Nurse Manager and state office
- Mortality Reviewer completes Final Mortality Report and facilitates Mortality Review Committee meeting within 30 days of receiving records
- Service recipient transitions and transfers
