POSITION SUMMARY
Primary function is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence, coordinate care with the Interdisciplinary Group/Team, patient, family, and referring agency.
POSITION DUTIES AND RESPONSIBILITIES
- Holistic Assessment:
- Collaborates with the RN in assessing the patient’s comprehensive needs, which encompasses the physical, emotional, psychosocial, and environmental dimensions. Documents findings diligently in clinical records.
- Care Planning & Implementation:
- Engages actively in the development and execution of care plans in sync with the RN. Ensures interventions are based on the patient's unique Plan of Care (POC).
- Infection Control:
- Upholds rigorous standards of infection control, integrating the principles of standard precautions to avert disease transmission and ensure a safe environment for both the patient and care team.
- Continuity & Quality of Care:
- Advocates for consistent and uninterrupted care for patients, emphasizing the delivery of high-quality, compassionate services.
- Effective Communication:
- Establishes open channels of communication, ensuring that patients, families, team members, and other healthcare professionals are always informed and aligned. Documentation, including clinical notes and evaluations, reflects this transparent communication.
- Regulatory & Compliance Monitoring:
- Oversees assigned patient cases to guarantee alignment with the stipulations set forth by third-party payors, ensuring that care is compliant and meets all regulatory requirements.

POSITION SKILLS AND QUALIFICATION
- Graduate of an accredited School of Vocational/Practical Nursing.
- State license as a Licensed Vocational/Practical nurse, or a multi-state license issued by a member of the National Nurse Licensure Compact (NLC). Current driver’s license.
- Two year experience as a Licensed Vocational/Practical Nurse in a clinical care setting, Hospice experience, preferred.
- Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.
- Should be CPR certified.
- Microsoft Office Suite Proficiency: Word and Excel.
- Reliable transportation. Valid and current auto liability insurance.
- Familiarity with platforms like Wellsky or Consolo to maintain and access patient information, care plans, medication histories, and more.
