Type: Full-Time: 40hrs/week
Salary: $18.31-$18.91/hr DOE

Key Responsibilities:
- Client Assistance: Assists patients in the clinic setting to provide support and guidance on various resources and benefits.
- Program Communication: Explain the benefits of the Community Health program to clients/patients and how it can improve their wellbeing.
- Health Management: Help patients identify socio-economic issues affecting their health and develop management plans.
- Documentation: Maintain comprehensive electronic client files, documenting encounters, referrals, and outcomes.
- Service Coordination: Facilitate communication and coordinate services between providers and clients to ensure effective care.
What We’re Looking For:
- Proficient computer skills, including internet-based applications
- Ability to thrive in a fast-paced environment while managing multiple priorities
- Strong organizational skills and accountability for time and activities with remote supervision
- A passion for serving others with professionalism and clear boundaries
- Community involvement or membership in the community served
Requirements:
- High School Diploma or GED
- Driver's license
Additional Certifications/Trainings upon hire:
- BLS Certificate
- Community Health Worker Certificate within the first six months of employment
- Certified Application's Counselor (CAC) within one year of employment
