Case Management Specialist

Job Title: Case Management Specialist

Reports To:

Status: Full-Time FLSA Exempt

Salary: $40,000-$45,000, commensurate with experience

Direct Reports: None

Hours: M-F 8:00 am to 5:00 pm, may require late & weekend hours based on organizational needs.

Work Site: One Place Family Justice Center 530 S. Lawrence Street Montgomery, AL

Job Purpose: The work of the Case Manager is to help victims achieve wellness and autonomy. The job involves assessing the individual’s needs, working collaboratively with them to develop a care plan that links them with services and resources within the community, advocating on their behalf, monitoring progress and staying connected with the individual until they can be discharged from OPFJC.

Major Tasks and Duties

  • Provide crisis intervention to calm and focus clients who are experiencing an episode of distress.
  • Conduct client assessments to understand the victimization, current living situation and to determine the help needed. The assessment will address the need for food, shelter, clothing, transportation, medical attention, emergency legal and criminal justice assistance and safety.
  • Educate the client about service options available within the community and partners with them to develop a practical care plan related to the problems created by the victimization.
  • Distribute security devices and other accessories based on the client’s attestation of safety concerns.
  • Maintain accurate case records documenting at minimum: the client assessment, care plan, contacts with community referrals, signed consent forms, progress notes demonstrating attainment of goals, discharge plan, security devices and other items provided to clients, and information shared by partnering agencies.
  • Make prompt referrals to partner organizations and follow up to ensure that the client received appropriate services.
  • Schedule follow up appointments in-person, by phone or audio/video technology to provide encouragement, support and monitor progress.
  • Participates in marketing and outreach activities as assigned.
  • Provides phone coverage at the front desk to answer and direct callers.
  • Attends applicable meetings, training sessions, and carries out other duties that may be assigned.

Knowledge, Skills and Abilities

  • Knowledge of and experience in the field of domestic violence.
  • Excellent public speaking skills and ability to provide education and training in formal and informal settings.
  • Effective verbal and written communication skills. Ability to inspire and motivate participants and help them overcome barriers. Bilingual (English/Spanish) preferred.
  • Proficient in Microsoft Office Suite.
  • Ability to maintain clear, sufficient case notes on all clients.
  • Familiarity with an ethnically/socially diverse community, and a variety of mentoring concepts and techniques.
  • Willing to work non-traditional hours and days to meet the needs of this position.
  • Ability to work well with a diverse population.

Minimum Qualifications

  • A minimum of a bachelor’s degree in social work, psychology, sociology, human services or related field. Work experience may substitute required education on a year-by-year basis.
  • One year of prior case management experience is preferred but not required.
