Responsibilities
- Administers treatments and/or medication as ordered by the physician.
- Visits each residence according to prescribed schedule to evaluate the individuals’ physical conditions.
- Reviews all health care provided to individuals including, related materials, recording notes and findings in the electronic medical record system and in the health care records.
- Participates in interdisciplinary team meetings.
- Acts as a liaison between the Individuals and the providers of health care in the community as well as with the consumer’s family and with other professional and para-professional staff.
- Performs assessments, problem identification & resolution and evaluation of the consumer’s health care needs.
- Agree to follow federal, state and local governmental regulatory guidelines pertaining to a safe, healthy and clean work environment, reporting health and safety concerns to the supervisor.
- Complete all tasks deemed necessary to provide optimal health care to Opengate Individuals.
- Performs other duties as assigned
Qualifications
- Must have Registered Nurse License in New York State
- Must have Drivers License in Tri-State Area (NY,NJ,CT)
- (1) year experience working with people with Intellectual and Developmental Disabilities
- Technical expertise in rendering nursing care
- Ability to work a flexible schedule.
Salary - $83,000 - $100,000 based on experience.
