Role summary
Optimum Wound Care is seeking a compassionate and skilled LVN to deliver mobile wound care in skilled nursing facilities, long-term care centers, assisted living facilities, and private homes. This role fits LVNs who enjoy patient interaction, field autonomy, and a flexible schedule outside a clinic setting.
What you will do
- Provide wound care treatments according to established care plans
- Perform dressing changes and wound care procedures
- Assist with wound assessments and monitor healing progress
- Document visits, progress, and care provided accurately and on time
- Communicate with patients, families, and facility staff to keep care aligned
- Educate patients and caregivers on wound care and prevention
Requirements
- Active California LVN license
- Valid driver’s license and reliable transportation
- Wound care, home health, geriatrics, SNF, or assisted living experience preferred
- Strong clinical judgment, professionalism, and attention to detail
- Excellent communication and time-management skills
